If you are considering participating in the ZORALCS-study, click the green button ‘I am considering participating’.
You then log in with the identification platform itsme®, and then complete the online questionnaire. Of course, registration is secure and we respect your privacy, as also described in our privacy statement. Succes!
Is logging in with itsme® difficult? Then take a look at the different steps below. You can also always visit a ‘digipunt’ in your municipality. They will be happy to help you with any digital questions. All healthcare providers from the ZORA primary care zone are also at your service. If you feel safer filling it in together with them, you can certainly do that too!
Step 1 – After the “I am considering participating” button, click “Log in with eHealth”
Step 2 – Click on “Sign in via itsme®”
Step 3 – Enter your cell phone number and click “Submit”
Step 4 – Confirm enrollment in the itsme® app on your cell phone
Step 5 – Click on “Confirm Profile.” Then you will automatically be shown the questionnaire.
